Home and Community Care Services

Home and Community Care Services (HACC) aims to help older people, people with a disability and carers maintain their independence.

Eligibility and Assessment

A formal assessment must be completed for access to Home and Community Care Services. A Council assessment officer will complete an assessment in the home. This takes around one hour. Access to services will be discussed during the home visit.

Services Available

Domestic assistance:
Includes services such as vacuuming, mopping and household cleaning. Shopping for household needs may be included.

Property Maintenance:
Includes minor household repairs which do not need a qualified tradesperson.

  • Carpentry and painting
  • Changing light bulbs where a ladder is required
  • Replacing tap washers
  • Unblocking drains
  • Cleaning guttering
  • Removing rubbish.

Personal Care:
Assistance can be provided for;

  • Bathing, dressing and grooming.
  • Mobility, (in and out of bed), transferring to a commode, wheelchair, chair or vehicle
  • Eating and drinking;
  • Toileting

Respite Care:
HACC provides individuals and carers with a break from their carers’ role. Respite can be provided in the home or in the community.

Paying for your HACC account

In person: Detach the payment slip on the invoice and bring it with your payment to the Nillumbik Shire Council offices. You can pay by cash, Eftpos - debit card, credit card or cheque.

By phone: Using only a credit card (the minimum amount for credit card $10.00)  Ph: 9433 3319

By mail: Detach the payment slip on the invoice and mail it with a cheque or money order made payable to Nillumbik Shire Council.

Delivered meals

Nutritious cooked meals cater for special dietary and cultural requirements. They are delivered chilled to your home Monday to Friday. Meals can be ordered for Saturday and Sunday. A fee applies for this service.

Telelink

Telelink is a telephone system that allows up to ten people to join a group discussion simultaneously. It provides an opportunity to make friends, learn from others and participate in group discussions.

  • There is no charge.
  • You do not need special equipment to join in.
  • Equipment can be provided free if you have trouble hearing or holding a phone.
