Commonwealth Home Support Program (previously called HACC)

The Commonwealth Home Support Program (CHSP) provides a strategy for delivering small amounts of timely low level home support services to a number of frail, older people and older people with a disability living in the community to maximise their independence

Council works with a range of disability and aged care service providers, health agencies, government departments and community members to achieve this outcome

My Aged Care

My Aged Care helps you find the information you need about aged care services

This can be as simple as calling the My Aged Care contact centre on 1800 200 422 or visit the My Aged Care website link listed below under referrals

My Aged Care provides you with information about:

  • different types of aged care services
  • eligibility for services

The contact centre staff will ask questions to determine your needs.  A referral will then be sent to either the Aged Care Assessment Service (ACAS) or to the Regional Assessment Service (RAS)

You will then be contacted to make a time for a trained Home Support Assessor to visit with you to discuss your needs and the services you require

Referrals

Referrals to the CHSP can be made by individuals, family members, general practitioners, the Aged Care Assessment Services (ACAS) and community service agencies. Please note, the person being referred must be aware of, and agree to, the referral

Referrals must be made through the My Aged Care contact centre. To make a referral please contact My Aged Care on 1800 200 422 or www.myagedcare.gov.au

Regional Assessment Service (RAS)

The Regional Assessment Service (RAS) is for older people who have low level needs. This means that they can remain in their home, but need some assistance with daily activities such as house cleaning or shopping

The RAS is conducted by a trained Home Support Assessor to find out what your aged care needs are. This is a face to face assessment conducted in your home

Commonwealth Home Support Services Available

To access these services a referral must be made through My Aged Care 

Personal Care is to provide frail, older people with support in activities of daily living that help them maintain appropriate standards of hygiene and grooming

Domestic Assistance is to provide frail, older people with assistance with domestic chores to maintain their capacity to manage everyday activities in a safe, secure and healthy home environment 

Flexible Respite is to support and maintain care relationships between carers and clients, through providing good quality respite care for frail, older people so that regular carers can take a break

Home Maintenance is to provide services that assists clients to maintain their home in a safe and habitable condition. Home maintenance services provided to clients must focus on repairs and maintenance of the home and garden to improve safety, accessibility and independence within the home environment for the client, by minimising environmental health and safety hazards

Home Modifications are to assist eligible clients with a service that will increase or maintain the level of independence, safety, accessibility and wellbeing

Meals is to provide frail, older people with access to meals

Other Food Services are to educate, train and reskill frail, older people in preparing and cooking a meal in their own home to promote their independence

Costs for Services

Services are funded by Council, State and Commonwealth Governments. Fees for services are means tested and are discussed once a referral has been made
