Commonwealth Home Support Programme

The Commonwealth Home Support Programme (CHSP) provides funding for a broad range of entry-level support services to assist older people aged 65 years and over (50 years and over for Aboriginal and Torres Strait Islander people) and who have functional limitations (including cognitive), to remain living independently at home and in their community.

CHSP services are delivered on a needs basis, with a strong focus on activities that support independence and social connections and take into account each person’s individual goals, preferences and choices.

 

CHSP Services

Personal Care is to provide, older people with support in activities of daily living that help them maintain appropriate standards of hygiene and grooming

Domestic Assistance is to provide, older people with assistance with domestic chores to maintain their capacity to manage everyday activities in a safe, secure and healthy home environment 

Flexible Respite is to support and maintain care relationships between carers and clients, through providing good quality respite care for frail, older people so that regular carers can take a break

Home Maintenance is to provide services that assists clients to maintain their home in a safe and habitable condition. Home maintenance services provided to clients must focus on repairs and maintenance of the home and garden to improve safety, accessibility and independence within the home environment for the client, by minimising environmental health and safety hazards

Home Modifications are to assist eligible clients with a service that will increase or maintain the level of independence, safety, accessibility and wellbeing

Meals is to provide frail, older people with access to meals

Other Food Services are to educate, train and reskill frail, older people in preparing and cooking a meal in their own home to promote their independence

Social Support Individual is to assist frail older people to participate in community life and feel socially included through meeting their needs for social contact and company.

Social Support Group is to assist frail older people to participate in the community life and feel socially included through structured, group-based activity that develop, maintain and support independent living and social interactions.

Transport provides an affordable door-to-door transport service for people who are elderly, people with a disability or their carers to access the community. See the transport page for details.

This can be as simple as calling the My Aged Care contact centre on 1800 200 422 or visit the My Aged Care website. www.myagedcare.gov.au

My Aged Care provides you with information about

  • different types of aged care services
  • eligibility for services

The contact centre staff will ask questions to determine your needs.  A referral will then be sent to either the Aged Care Assessment Service (ACAS) or to the Regional Assessment Service (RAS)

You will then be contacted to make a time for a trained Home Support Assessor to visit with you to discuss your needs and the services you require

Costs for Services

Services are funded by Council and Commonwealth Governments. Fees for services are means tested and are discussed once a referral has been made
