Advance Care Planning is about making your wishes known for future health care, should you be unable to speak for yourself.
This interactive workshop will help participants understand what advance care planning is, why it matters, and how to get started. We’ll discuss values, goals of care, choosing a substitute decision‑maker, and the role of advance care directives.
The session is designed to empower individuals to have meaningful conversations with loved ones and health professionals, ensuring their preferences are known, respected, and documented.
Advance Care Planning documents will be available for participants to start working through, brainstorm and explore with the Advance Care Planning team.
